Chronic Condition Special Needs Plans are a Medicare Advantage plan type that cares for the needs of those with certain chronic conditions. They have networks of healthcare providers who treat that condition and include prescription drug coverage.

Chronic Care SNPs have networks of healthcare providers and specialists who treat the chronic conditions of their members. The specialist that may have been outside of your plan network and thus outside of your price range is now accessible with a C-SNP.
